Taylor Swift fans spot unexpected celebrity cameo in new music video at the 2026 VMAs
An unexpected famous face has made a shock cameo on Taylor Swift's new 'Patient Zero' music video, which just premiered at the 2026 VMA Awards.
The 36-year-old hitmaker has officially made history by winning becoming the most-awarded artist in VMA history, breaking her tie with Beyoncé.
During the Los Angeles ceremony on Sunday (27 September), she was presented with the first-ever award for Artist Director by her pal Dakota Johnson.
The broadcasters said the artist director honour is a ‘new award recognising groundbreaking music artists whose work behind the camera has expanded the possibilities of music video and advanced the art of visual storytelling’.

After a quick speech, she gave the people what they wanted and premiered the brand-new video for 'Patient Zero', which was released on Friday (25 September) and stars Johnson and Colin Farrell.
However, as we say, although fans knew the acting duo would be in the video, there was a third celebrity that nobody knew would be making an appearance - Cara Delevingne.
The 34-year-old was spotted by fans sitting behind Tay and Dakota in one awards show-style scene of the music video.
Taking to X to react, one excited viewer penned: CARA HAVING A CAMEO OH TAYCARA IS ALIVE AND WELL #VMAs."
While a second added: "Was just thinking about how 1989 coded the patient zero mv is then Cara showed up omg."
A third wrote: "Cara Delevingne being in the mv had me shooketh" and a fifth penned: "I KNEW IT WAS CARA DELEVINGNE!!! I WASN’T SURE AT FIRST BUT OMG!!!!!"
Meanwhile, other fans have simply been gushing over the 'incredible' video, with some even going as far as to say it's Swift's best yet.
"I didn’t cry listening to Patient Zero, but I did cry watching the music video. That was one of her best videos yet, SO stunning. ouch! #vmas," one fan penned, while a second added: "Patient Zero video makes the song hit different. Wow, what a video and a message #VMAs.
And a third and fourth wrote: "OMG the Patient Zero video was EVERYTHING!!!! I'm shook #VMAs" and, "Patient Zero isn’t my fav Taylor song but that might be my favorite music video of all time holy that was so well done #vmas."
Swift and Delevingne have been close friends for about twelve years, with the actress previously featuring in the singer's 'Bad Blood' music video, which was released in May 2015.
The Paper Towns star previously revealed that, at one point, she moved in with Swift following a rough breakup.
She told Interview Magazine in November 2024: "We’re very different people. She’s very homely, because she looked after me so well, but we got into some - not trouble, but I definitely took her for a bit of a wild ride."
In a new message on Instagram following the video's release, Swift wrote: "The Patient Zero video is finally out in the world! Getting to work with @chivexp was something out of a dream sequence, and I learned so much from getting to watch his genius in action.
"I want to thank @dakotajohnson and Colin for trusting me with their time, taste, and talent (Giggling with your friend because you’re twinning for an entire shoot was a new and truly delightful experience). And Colin’s turn as a villain is even more impressive when you get to be around him. He’s an absolute light and as kind as they come.
She continued: "@caradelevingne, thank you for agreeing to play the lusty other woman. That’s what friends are for?
"With the Patient Zero video, I got to explore the cinematic and literary genres I love so much. I wanted to incorporate elements of gothic ghost stories, true crime, psychological thrillers all in a setting I hadn’t really shot in much.
"The house was a metaphor for the coldness of a relationship that’s faded and atrophied.
"Thank you to @ethantobman for your insightful production design, and bringing that feeling to life. My editor @chancler has outdone himself. And lastly, thank you to the @vmas for giving us a World Premiere at the place where celebrating music videos became a thing. And so much love for you guys, always."
