You can fill out the accompanying form here.
Given that Derren previous shows have featured stunts like the famous £100,000 "armed robbery" and an attempt at art theft, plus a whole lot of hypnotism, it's anyone's guess what you'll be signing up for - but if that doesn't faze you, go for it!
As well as details of your hobbies, interests and occupation, you'll need to answer questions like: what makes you a perfect fit for one of Derren's shows? Why do you love Derren? And, what's been your preferred Derren show to date?
Better get brainstorming some intriguing answers, then.
As yet, we don't have further details of the show or an air date - but we think it's safe to say it'll leave viewers reeling, and we'll update you as soon as we know more.
Derren fans also have his SHOWMAN UK Tour - his first theatre show in SIX years - to look forward to.
The tour - which was postponed due to the coronavirus crisis but is now set to premiere in February 2021 - will see Derren return to astonish audiences up and down the country.

Talking about SHOWMAN, Derren said: "We were all ready to open Showman when the tour had to be postponed.
"Now, after sitting around in my pants for months I am so, so eager to get this show on the road. I'm hugely grateful to everyone who bought tickets and then were told they had to wait. Let's get together and have a hug and party."
"All I can reveal at this point is that the show will ultimately be about you, the audience member, because that's what I find most interesting."
The illusionist continued: "There are places I'd like to take you where we haven't been before. I always aim to have it deliver more than you'd expect.
"I'm excited to be at the starting point of that process. Getting it on the road will be my favourite part of the year."
